Prescription Medication Form

Prescription medications must be handled and administered properly from owner to pet.  Medications for the heart such as Rx Heartguard and Rx Tri Heart can have detrimental affects on circulation.  There is no exception to this rule, and even medications such as Rx Mometamax Otic Suspension for the ear and Rx Clavamox Tabs for infection must be administered properly to protect the health of your pet.  If you have any questions regarding the dosage or frequency in use of these medications please contact your local veterinarian.  Pup’s Pet Supplies is not responsible for any misuse or ill effects due to negligent administration of prescription medications for your pets. 

Prescription drugs/medications are restricted to use by or on the order of a licensed veterinarian. Before we can process your order, we must have a prescription from your veterinarian on file. For your convenience, we have three (3) ways you can do this:

1. By Fax: Your veterinarian's office may fax your prescription to: (402) 223-4433

2. By Phone: Your veterinarian may call in your prescription to us at 1-877-787-7123, 8am to 5pm Central Time, Monday-Friday. Your veterinarian may leave us a voice message after hours.

3. By Mail: You may mail the original written prescription to us at:
